REFERRALS & PLACEMENTS

Finding the right home for every young person

Every referral is considered individually. We work collaboratively with social workers, commissioning teams and other professionals to understand the young person's needs and

determine whether one of our homes could provide the right environment.

1. Initial enquiry

Contact our team to discuss the young person and the type of placement being sought. Where appropriate, we will request relevant referral information so that we can begin considering suitability.

2. Review of information

We review the information available to understand current circumstances, care history, identified risks, emotional and behavioural needs, education, health, relationships, strengths, interests and placement objectives.

​

3. Matching & impact assessment

We consider the referred young person's needs alongside the needs and dynamics of young people already living within the home. A vacancy does not automatically mean that a placement is suitable.

4. Placement discussion

Where we believe we may be able to meet the young person's needs, we discuss the proposed placement further with the placing authority, including expectations, risks, care requirements and intended outcomes.

5. Planning the move

Where a placement is agreed, we work with the placing authority and relevant professionals to plan the transition. Where circumstances allow, we aim to make the move as thoughtful and supportive as possible. Information to provide where available and appropriate, send the referral/placement request, current care plan, risk assessments,

placement history, education and health information, relevant assessments, significant relationship information, current professional involvement and placement objectives.
